Dental appointment card
A pink coloured Dental appointment card. Printed on the card are the words "NORMAN J DONOVAN B.D.S ENG & ASSOCIATES DENTAL SURGEONS". Also printed on the card is their address "SILVERTOWN DENTAL SURGERY SAVILLE ROAD SILVERTOWN E.16 TEL. 01-476 1853-2300". Written above this adress in blue ink is the name "MR SORATUR". There is a blank field underneath with space to write when the person's next appointment with the dentist would be. Underneath the field is adisclaimer saying that a fee will be charged for cancelled or broken appointments without 24 hours notice and for the appointee to bring the card with them.
