Three buttons.
Two have a twisted rope edge. On a circle with a lined background is an oval with a smooth background and a twisted rope around the outer edge of the oval. On the oval is an anchor with a snake twisted around it, the head poking left through the ring/shackle. The head is poking left through the ring/shackle at the top. Its body goes under the right-hand stock, over the shank to the left then under the shank to the right, over the right arm and curls under the crown. Surgeon

Its body goes under the left-hand stock, then over the shank to the right, then under the shank to the left and under the left arm. Surgeon.
